Immigration Examination

We perform physicals by appointment only Monday through Friday only. At the scheduled time, be sure to bring your vaccine card and government identification with you. If you cannot understand, read, and write in English, you need to bring a family member or translator with you. We do not bill insurance companies. As for payment options, we accept cash and credit cards, with the cost of the service due before your examination. There is a $25 required deposit to hold your appointment that is given to you as credit at the time of your examination.

Vaccine requirements:

• In the past, the USCIS organization will change their requirements and our office is not notified of these changes. While we frequently monitor the USCIS website frequently for such changes, please always confirm which vaccines you will need. Thank you.
• Must have proof of immunization with a vaccine card or reported date of the disease such as chicken pox. If vaccines are not documented, the following vaccines are required and we do carry most of them (please see bottom if chart difficult to discern):

¹ Must have had a polio vaccine within 12 months of the immigration physical (starting from 2/23/2023); IPV acceptable 6 weeks on

² If you have a history of chicken pox, please provide the year and then you will not need the vaccine.

³ Flu/influenza shots required Oct 1^st – March 31^st

⁴ Children 7-10 years old sometimes need a dose of Tdap depending on their vaccine history.

7 years – 10 years: Td/Tdap ($65)(depends), Polio ($55), MMR ($55), Hep A ($95), Hep B ($75), Varicella ($150), influenza ($45)(Oct 1^st – March 31^st)

11 years – 18 years: Td/Tdap ($65), Polio ($55), MMR ($55), Hep A ($95), Hep B ($75), Meningococcal (pharmacy), Varicella ($150), influenza ($45)(Oct 1^st – March 31^st)

18 years – 59 years: Td/Tdap ($65), Polio ($55), MMR ($55), Hep B ($75), Varicella ($150), influenza ($45)(Oct 1^st – March 31^st)
